Illness System

Illnesses can develop through normal Trenchport gameplay, close contact with contagious players, dangerous heat, animal handling, or the private Intimacy sexual-health flow. This guide explains what can happen, how exposure works, and what players can do after symptoms begin.

Background wellness & medical system

Exposure → Symptoms → Diagnosis → Treatment

With Wellness enabled, the Trenchport cloud can record background illness exposure from qualifying gameplay. Automatic conditions begin hidden and incubating. Once symptoms appear, the HUD gives private symptom information without immediately naming the condition. Hospital diagnosis reveals what the character has and which treatment applies.

Wellness must be enabled

Automatic gameplay exposure is disabled when Wellness is off. Staff-created medical conditions and private STI processing follow their own medical flows.

Automatic illness is limited

A player can have only one active/incubating automatic condition and can receive only one new automatic condition in a rolling seven-day period.

Recovery gives protection

After recovering from an automatic condition, that same condition has a 30-day automatic exposure immunity window.

Current Exposure Routes

Not every catalog illness is attached to a random background trigger. The routes below are the current automatic or private transmission paths used by the system.

Close contact

Contagious Proximity

Two Wellness-enabled current-HUD players must remain within 3 meters continuously for 60 seconds. Only an ACTIVE contagious condition can spread; incubating conditions do not spread yet.

Rummaging

Dirty & Food Exposure

Rummage results can carry sanitation or food-related risk.

  • Dirty-theme rummage: Bacterial Infection 2%
  • Kitchen, rotten, or food-related rummage: Food Poisoning 3%
Fishing

Catch & Fish Handling

Fishing contact and gutting can expose players to Bacterial Infection.

  • Hazardous catch/contact: 4%
  • 12+ catches within 24 hours: 2%
  • Fish gutting: min(8%, 2% + quantity)
Animal contact

Cock Handling & Bird Flu

Cock activities are animal-contact exposure only and do not add heat-exertion points.

  • Feed: 1%
  • Train: 2%
  • Fight: 3%
  • Slaughter: 5% and higher severity
Mobile-area weather

Heat & Activity

Weather alone never makes a player sick. The Trenchport cloud combines real Mobile/KMOB heat conditions with exertion from rummaging, fishing, fish gutting, and combat/hunting.

  • Lower heat bands can trigger Dehydration after enough exertion.
  • Danger and Extreme Danger can escalate to Heat Exhaustion.
  • Recent recognized water lowers the effective exertion total and halves the final heat-illness chance.
Private sexual health

Intimacy & STIs

Chlamydia, Gonorrhea, and Syphilis are transmitted only through the completed private Intimacy sexual-health flow when a partner is affected. Condoms reduce risk; Birth Control Pills do not.

Crafting note: crafting can drain Hunger, Hydration, and Stamina, but it is not currently a seeded automatic illness source.

What Happens After Exposure?

Automatic conditions stay hidden by name while incubating and undiagnosed. Once incubation ends, symptoms become active but the diagnosis remains private until the medical system identifies the condition.

Treatment rules

Recovery & Medication

  • Natural recovery occurs when the condition reaches its expected recovery time.
  • Exact matching treatment items can shorten recovery and lower severity.
  • Hospital staff can treat through the medical terminal without taking the patient's inventory item.
  • The same condition has a minimum 6-hour treatment cooldown between mechanical treatment applications.
Heat protection

Water Matters

  • Recognized water grants 2 hours of recent-water heat protection.
  • Recent water reduces effective heat exertion and halves the final activity-related heat illness chance.
  • It prevents qualifying activity from prolonging an existing heat condition.
  • When used during Dehydration or Heat Exhaustion, water can shorten remaining recovery, reduce severity, and cut stored penalties.
Ambient roleplay

Symptoms Can Be Heard

When Ambient Symptoms are enabled in Config, active respiratory/allergy conditions can drive automatic coughing, sneezing, or sniffing behavior through the HUD. Turning Ambient Symptoms off silences those automatic RP sounds without curing the illness or removing its mechanical effects.
